JOB DESCRIPTION

The LPN (Licensed Practical Nurse) will provide direct client care,
monitor health conditions, and assist with daily living activities under
the supervision of the Director or Administration. They play a vital role
in ensuring the well-being and safety of our clients.

QUALIFICATIONS:

• Must possess a current, unencumbered, active state license to practice as an

LPN in the state of FL

• Must be CPR/BLS certified

• Experience in Skilled Nursing and elder care

• Knowledge of common conditions affecting the elder such as Alzheimer’s

and other dementias

• Must be qualified, compassionate, and dedicated

• Strong communication skills

GENERAL DUTIES AND RESPONSIBILITIES:

• Assist in the implementation of individualized treatment plans for each

assigned client

• Provide nursing services to clients

• Supervise CNA’s in providing personal care to clients

• Provide personal care, assisting with but not limited to; walking, toileting,

diet management and eating

• Administer medication(s) and treatment(s) as prescribed by physicians

• Monitoring vital signs

• Providing first aid treatment and assist RN with responding to emergency

medical situations

• Report clients’ concerns to Administration

• Strong communication skills

• Other duties as assigned by the Director or Administrator
